Waiver & Release of Liability (General Mobile Services)

Acknowledgements

I understand and agree that:

• Labpreps provides mobile healthcare services such as mobile phlebotomy/specimen collection, urine sample collection, and home injections as ordered/prescribed.

• As with any healthcare service, there are potential risks, including but not limited to, bruising, bleeding, or swelling at the venipuncture site, pain or discomfort during or after the blood draw. dizziness/fainting

(related to blood collection) infection, and/or hematoma (rare but not possible)

• For injections, risks may include soreness, swelling, redness, infection, or allergic reaction. In rare cases, more serious reactions may occur.

• I have disclosed relevant medical history (including allergies, bleeding disorders, and medications like blood thinners) to the best of my ability.

• I may stop or refuse services at any time.

Release

I have read, underdstood, and agreed to the terms and conditions outlined in this service agreement. To the fullest extent permitted by law, I release and hold harmless LabPreps and it's staff/contractors from claims arising out of the mobile service visit except to the extent caused by gross negligence or willful misconduct.
